As part of our commitment to provide more personalised ongoing care, all patients have a named accountable GP.

The named GP will:

• Take lead responsibility for your care and ensure all appropriate services required under the contract with the practice are delivered to you (Patients do not always need to see their named GP when requesting an appointment with the practice).
• Work with relevant health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient.
• Ensure that your physical and psychological needs are recognised and responded to by the relevant clinicians at the practice.

All patients will be given a named GP at the time of registration with the practice. 

The above does not affect your right to choose to see a GP of your choice.
